Frail Care
Our frail care service is dedicated to providing comprehensive support for elderly or physically frail individuals. Our compassionate caregivers assist with personal care, meal preparation, medication management, and mobility support, ensuring your loved one's well-being and comfort.
How our caregivers help
Every plan is person-family-centred and co-created with you, your family and our case managers, so the support below is shaped around the individual.
- Personal care
- Meal preparation
- Medication management
- Mobility support
- Companionship and reassurance
- Regular updates for family
This service suits
- Elderly people who need daily support at home
- Physically frail individuals recovering strength
- Families who need relief from full-time caring
How we deliver Frail Care
Frail care works best when it feels like an extension of family life, not a hospital routine. Our caregivers get to know your loved one, their habits and their preferences, and build each visit around what helps them feel comfortable and dignified. Personal care is handled with patience and privacy, meals are prepared to suit dietary needs, and medication is given on time and recorded. Where mobility is limited, we help with safe transfers and gentle movement so that your loved one stays as active as they are able.
Signs that extra support may help
- Everyday tasks such as bathing, dressing or cooking have become difficult
- There have been falls or a fear of falling at home
- Medication is being missed or doubled up
- Family carers are stretched and need reliable support
Getting started is simple
- 01 Get in touch Send a booking request, call or message us on WhatsApp. We listen to what is happening at home and what kind of support would help.
- 02 Co-create a plan Our case managers meet with the individual and their family to build a person-family-centred support plan, with nurses overseeing health needs.
- 03 Care begins A caregiver matched to the plan starts visits. We review the plan with you as needs change, so support always fits.
